Kindergarten Registration
It's time once again for Kindergarten Registration. Whether your child is starting school in the Fall or attending a new school, make sure you're prepared with the proper forms. Most schools require a Statement of Health and Immunization Record for enrollment.
Due to the volume of requests, please allow 24-48 hours for processing.
Statement of Health completion requires a child to have had a Well Child Visit in the last year. Please ask a Receptionist if you're unsure of the date of your child's last Well Child Visit.
